The US Occupational Health & Safety Administration (OSHA) estimates that in the US nearly 100 workers die every year in forklift related accidents with 20,000 more being seriously injured.
This means every 3 days a forklift accident occurs that kills somebody in the US.
OSHA also reports that almost 80% of all forklift accidents involve a pedestrian.
Canadian equivalent stats are not easily available but it is likely that in percentage terms the numbers are very similar.
