Tasmania – a 24-year-old man was unloading a truck with a forklift when the forklift tipped onto its side, pinning him underneath. He was killed.
San Diego – a 68-year-old employee at an auto-parts business was found wounded near a forklift with its engine running; authorities concluded that the victim had been run over by the vehicle. He died at the scene.
Reno — a man was crushed by a six-ton rental forklift he was picking up at an Amazon warehouse. Reports say that he had loaded the forklift onto a flatbed truck but when he started lowering the truck bed the unsecured forklift fell onto him. He was killed.
